Trigeminal neuralgia — lightning-like, unilateral facial pain attacks in the supply area of the trigeminal nerve
Trigeminal neuralgia

Lightning-like facial pain — and an approach at its source.

A trigeminal pain comes like an electric shock — seconds, but overwhelming, triggered by a touch, a breath of wind. As an Axomera therapy reference centre, we have been treating trigeminal neuralgia for years and pursue the goal of relief up to complete remission — especially in cases of insufficient response to medication.

Why patients come to us

We know your path.

Many of our patients have years behind them: misdiagnoses, many changes of doctor, medication with side effects to the limit of what is bearable — and despite this, continued pain attacks.

We check your medical history carefully first, before we suggest Axomera therapy. Due to the treatment results observed by us, patients travel for this from all over Germany, Austria and Switzerland — especially for trigeminal neuralgia.

Patients

What patients report.

Typical findings

From our patient consultations and the medical reports submitted.

  • Lightning-like second-attacks, often up to 100 times or more per day
  • Triggers: light touch, chewing, speaking or cold air
  • Unilateral pain in forehead, cheek or chin — in the supply area of the trigeminal nerve
  • Often consulted several doctors, in some cases only correctly classified after years
  • Insufficient response to medication or burdensome side effects
Previous therapy

Almost always in this order.

  1. Antiepileptics (e.g. carbamazepine) as basic therapy
  2. Dose increase or combination of several antiepileptics
  3. Additional analgesics in case of insufficient effect
  4. In case of no response: clarification of a vascular-nerve contact
  5. Surgical procedure: microvascular decompression according to Jannetta
Clinical picture · Classification

An independent facial pain illness.

According to the International Classification of Headache Disorders (ICHD-3), trigeminal neuralgia is an independent diagnosis in the chapter “Painful lesions of the cranial nerves and other facial pains” — separated both from the trigeminal autonomic cephalalgias like cluster headache and SUNCT syndrome and from painful trigeminal neuropathy, an independent, related diagnosis in this chapter. The clean classification is crucial because the attack character, accompanying symptoms and treatment of these clinical pictures differ.

Trigeminal neuralgia — what is conventionally possible

Our approach

How we proceed with you.

Every therapy needs its own examination — for us, this includes the Axomera diagnostics and the Axomera therapy.

Axomera diagnostics
  1. Confirm the diagnosis and check previous medication
  2. Millimetre-precise examination — localising the exit points of the trigeminal nerve
Axomera therapy
  1. 4–6 Axomera treatments, targeted at these points
  2. Goal: significant reduction in attack frequency and intensity
  3. In favourable courses: symptom-free remission observed over years

Aimed goals. We cannot guarantee a specific result — progressions are individual.

Where we make the difference

Axomera — a neuromodulatory procedure.

We know the standard drug therapy and its limits from daily practice; where operative decompression is sensible, we address this openly. Our focus, from years of experience with facial pain conditions like trigeminal neuralgia, is percutaneous bioelectric stimulation (PBCS) — here we explain how Axomera is applied for trigeminal neuralgia.

A nerve that is damaged or irritated at one point becomes hypersensitive not only there but throughout its entire course. This is known from other nerves: in the case of the sciatic nerve, even stretching far from the actual damage triggers pain. In the face, the same hypersensitivity manifests as a trigger point — a light touch on a small spot, for instance, right next to the nostril, can trigger an entire pain attack. Such points are accessible trigger points of the overstimulated nerve.

Axomera is a neuromodulatory procedure: computer-controlled electrical signals in the dimension of cellular electrical fields, placed with millimetre precision at exactly these trigger points, can calm the overstimulation there and raise the threshold from which an attack is triggered. As patient progressions show, the irritated nerve region can calm down and normalise over time under this treatment (remission). This neurophysiological hypothesis is empirically founded by many patient progressions (real-world data), not by controlled trials; as with other indications, around 20–30% of those treated do not respond.

Inflammation has an electrical dimension. Axomera starts exactly there.

The goal is relief — in a favourable case, even symptom-free remission. How far this succeeds depends on the individual course.
