axomera
Greater trochanteric pain

Getting to the bottom of nocturnal hip pain.

The pain at the outer hip often flares up at night as soon as one lies on the side. As a reference centre for Axomera therapy, we examine the irritated tendon and bursa region at the greater trochanter and treat it bioelectrically.

For two years, bursitis in both hips; sports, walking, and sitting for long periods were severely restricted. Three doctors, X-ray, sonography, pain injections, physiotherapy, and tablets only helped for a few days. After the first Axomera therapy, it got significantly better after three days, six further treatments brought further improvement.

Clinical picture

Irritation at the greater trochanter.

The Greater trochanteric tendinosis is an irritation of the tendon insertions and the bursa at the greater trochanter of the femur (also known as trochanteric bursitis or greater trochanteric pain syndrome).

Causes are usually overuse and age-related decline in load-bearing capacity; women are affected more frequently and usually over the age of 60.

Patients

What patients report.

Typical findings

From our patient consultations and the medical reports submitted.

  • Complaints usually for several months to years
  • Often consulted multiple doctors — no lasting improvement
  • Well-localisable tenderness on the outside of the thigh
  • Nocturnal pain when lying on the affected side
  • Pain when climbing stairs and standing on one leg; rest provides relief
Previous therapy

Almost always in this order.

  1. Pain-killing and anti-inflammatory medication
  2. Physiotherapy (stretching, abductor strengthening)
  3. Shock wave
  4. Cortisone injection (bursa)
  5. Surgery (rare, last resort)
Where we make the difference

For greater trochanteric pain, management usually begins with rest, physiotherapy, cortisone injections into the bursa, and shockwave therapy — if irritation persists, surgical removal of the bursa is occasionally considered. Injections are often only temporarily effective, and repeated doses of cortisone can further weaken the tendon tissue.

Axomera for greater trochanteric pain.

At the greater trochanter of the femur, the attachments of the gluteal tendons (Mm. glutei) and a bursa meet. In the event of overload, such as from asymmetrical gait patterns or frequent lying on the side, the tendon remodelling process falls behind here, and the bursa becomes inflamed as a result. Because this structure lies directly under the skin, it is easily accessible from the outside — unlike, for example, a deeply situated nerve root.

Axomera targets precisely this region: computer-controlled electrical signals in the dimension of cellular electrical fields are placed with millimetre precision at the tendon attachment and bursa. The goal is to shift the disrupted tendon remodelling back in favour of healing — not just to mask the pain. This neurophysiological hypothesis is empirically founded on many patient case studies (real-world data), not on controlled trials; as with other indications, around 20–30% of those treated do not respond.

One millimeter decides whether the field is on the irritated tissue or next to it. That is why we perform an ultrasound beforehand.

Our approach

How we proceed with you.

Every therapy needs its own examination — for us, this includes the Axomera diagnostics and the Axomera therapy.

Axomera diagnostics
  1. Secure the diagnosis and check the influence of taken medication
  2. Millimetre-precise examination — localising pain points at tendon insertions and the bursa at the greater trochanter
Axomera therapy
  1. 4–6 Axomera treatments, targeted at these points
  2. Goal: significant reduction of pain and tendon remodeling disturbance
  3. In favourable cases: pain-free lying and stair climbing

Aims & course of treatment. We cannot guarantee a specific result — the progression is individual. Depending on the course of the condition, fewer or more treatments may be required.

Case 1 / 2
For about six years, pain in the left hip when lying down; injections and physiotherapy never fully helped, especially at night on my side. After three Axomera therapies — significantly better after just the first one — the pain disappeared.
